NYSBDC Data Survey

The New York State Bleeding Disorders Coalition (NYSBDC) is conducting an anonymous survey to collect demographic data and information on the needs of the bleeding disorders community across New York State. All of the information collected in this survey will be compiled into data points to be utilized solely for advocacy purposes by the NYSBDC.

We will randomly select an order number to win a $50 Amazon Gift Card. We will be posting the winner’s order number on the organization’s websites and social media, so make sure to keep an eye out for the announcement.

You must live in New York State and either have a bleeding disorder, be a carrier, or be a family member of someone with a bleeding disorder in order to fill out the survey.

This survey will take no more than 5-10 minutes to complete and will close on Friday, May 15th, 2020 at 11:59 PM ET.
