Photograph & Video Release Form/Autorización de uso de fotos y video
I hereby grant the New York City Hemophilia Chapter specific permission to reproduce, publish, circulate, copyright or otherwise use any and all photographs and/or videotape of me and/or my family taken today for use by NYCHC. I understand that my image may be edited, copied, exhibited, published or distributed and waive the right to inspect or approve the finished product wherein my likeness appears. There is no time limit on the validity of this release nor is there any geographic limitation on where these materials may be distributed. This release applies to photographic, audio or video recordings collected as part of the event listed on this document only. By signing this form I acknowledge that I have completely read and fully understand the above release and agree to be bound thereby. I hereby release any and all claims against any person or organization utilizing this material for educational purposes.
Por este medio, otorgo permiso específico al Capítulo de Hemofilia de la Ciudad de Nueva York para reproducir, publicar, distribuir, obtener derechos de autor, mostrar o utilizar de cualquier otra forma para todos y cada fotografía y / o video de mí y / o mi familia tomada hoy para uso de NYCHC. Entiendo que mi imagen puede editarse, copiarse, exhibirse, publicarse o distribuirse y renuncio el derecho de inspeccionar o aprobar el producto final en el que aparece mi imagen.
No hay límite de tiempo para la validez de esta autorización ni existe una limitación geográfica sobre dónde se pueden distribuir estos materiales.
